The Delaware County Health Department needs your feedback!

We kindly ask that you take 10-15 minutes to complete their Community Health Assessment Survey by Friday, June 30. Your input will help the DCHD and its partners identify health needs and priorities and aid in the development of a Community Health Improvement Plan.

The survey is open to those who are 18 and older who live, work, go to school, or receive services in Delaware County, and it can be completed in English, Spanish, Arabic, Bengali, Chinese, French, Punjabi, Urdu, and Vietnamese.
